Pursuant to Article 13(1) and (2) of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), (OJ of the EU.L 2016 no. 119, page1) hereinafter: “GDPR”, I hereby inform as follows:
I. Data Controller
Niezależna Fundacja Filmowa [The Independent Film Foundation] having its seat in Poland, Warszawa (01-670), ul. Marii Kazimiery 23E, is the Controller of your personal data.

II. Purposes and legal basis for personal data processing
Your personal data, such as name, surname, e-mail address, company name, function/position, shall be processed:
1) under Article 6(1)(a), (b) and (c) of the GDPR:
a) in order to create and broadcast live online transmission of the Krzysztof Kieślowski’s ScripTeast Prize-giving Ceremony (hereinafter: the “Ceremony”) via the social media site Facebook, and later – to distribute the whole or fragment(s) of the Ceremony transmission via audio-video access means and websites, e.g. YouTube;
b) in order to abide by legal obligations of the Controller.
2) under Article 6(1)(f) – in order to perform the Controller’s legitimate purpose in case of legal necessity of demonstrating facts, demonstrating the Administrator’s compliance with his obligations.
III. Period for which the personal data will be stored
Your personal data will be stored for the period necessary to meet the objectives specified in point II above; after that time they shall be stored for the period and within the scope as required by applicable law provisions.
IV. Recipients of the personal data
The Controller intends to make the personal data available to the following categories of data recipients: commercial and business partners. The data recipients shall process the data in the Controller’s name or in their own name – to distribute the whole or fragment(s) of the Ceremony transmission via audio-video access means and websites. In addition, those data can be made available to processing entities who render services to the data Controller and who are provided with the data concerned.
V. Rights of the Data Subject
Pursuant to the GDPR you have the following rights:
1) right of access to your data and right to receive copy thereof;
2) right to rectify (correct) the data;
3) right to erase the data;
4) right to restrict processing of the data
5) right to object to the processing.
6) right to lodge a complaint with the supervisory authority – Prezes Urzędu Ochrony Danych Osobowych [President of the Personal Data Protection Office], ul. Stawki 2, 00-193 Warszawa, should you claim that the Controller has violated law provisions on personal data protection;
7) right to withdraw your consent – as much as the consent constitutes the basis for processing your personal data you have the right to withdraw the consent. Withdrawal of the consent shall not affect the lawfulness of the processing performed pursuant to the consent before withdrawal thereof.
VI. Transfer of the personal data to third countries
Personal data provided by you shall not be transferred outside the European Union or European Economic Area.

The data shall not be subjected to automated decision-making, including profiling
VIII. Obligation to provide personal data
Your provision of personal data and consent for processing thereof is of voluntary nature, however provision of the data constitutes the condition for participation in the Ceremony.
Pursuant to Article 6(1)(a) of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), (OJ of the EU.L 2016 no. 119, page1), I hereby give my consent for processing my personal data within the scope of my name, surname, e-mail address, company name, function/position for the purpose of the online transmission of the Ceremony via the social media site Facebook, and for the purpose of later distribution of the whole or fragment(s) of the Ceremony transmission via audio-video access means and websites. In addition, I hereby declare that I made myself acquainted with the content of the information clause of the data Controller, i.e. Niezależna Fundacja Filmowa.
